While contact names and info are not required on the form below, providing such information helps the Core Collaboratives organization confirm the validity of feedback.  Customers who provide their name & contact info have an option to share this information with business representatives who are instructed to use the information to provide updates on services and/or concessions to customers with less than satisfactory experiences.
